(GERING, Neb.) — The Aging Office of Western Nebraska is hiring a Care Manager to assist elderly individuals needing nursing home care or assisted living, focusing on managing relationships, communication, organization, and action orientation.

Job Description:

Under the supervision of the CMU Supervisor, performs the assessment, development, implementation, authorization, processing, and evaluation of community-based services for the elderly throughout the Panhandle of Nebraska.

Examples of Work Performed:

1. Extensive assessment of potential elderly clientele and their needs to obtain or maintain community based in-home services.

2. Process and coordinate community-based services for the elderly and their families.

3. Implement a program assistance / care plan for the elderly who qualify under program guidelines.

4. Work in cooperation with Resource Development in the securement of appropriate service providers.

5. Authorize providers and services needed.

6. Evaluate applicable services and programs on a monthly / annual basis.

7. Acquire and maintain familiarity with appropriate laws, policies, regulations, and directives of the AOWN, Nebraska Health and Human Services, and other State and Federal agencies that the AOWN works directly with.

8. Maintain familiarity of community resources and provide appropriate information and assistance to elderly and families.

9. Attendance at programmatic meetings.

10. Participation in training as directed and approved by the CMU Supervisor or Director.

11. Participation in training as required by programmatic contracts.

12. Maintain confidentiality of all client information.

13. Entry and verification of client information in the agency’s automated reporting and tracking system (Peer Place).

14. Maintenance of records as directed.

15. Completion of monthly reports as directed.

16. Provide input to periodic quality assurance reviews.

17. Assist in the orientation of service providers, program participants, and other community agencies or groups.

18. Serve as an advocate for participants and clientele.

19. Work “on-call” as necessary.

20. Other related duties applicable to the position as assigned by the CMU Supervisor or Director.

Pay: $20.00 – $22.00 per hour
